What is Partners in Recovery?

If you have a serious and persistent mental health issue with a range of complex needs, Partners in Recovery can help.
We’ll meet with you to assess your needs and understand your hopes and life goals. We then link you into a range of services and supports to help you meet those goals. 
You will be assisted by a dedicated Support Facilitator to identify any gaps in your care and work with you to develop your personalised recovery plan. 
Funded by the Department of Health, 360 Health + Community is the lead agency for the PIR program in the Midwest and Goldfields regions of Western Australia.
360 work in collaboration with a network of organisations across WA to deliver the best outcomes for our participants. These are Mission Australia, Bay of Isles Community Outreach (BOICO) and Hope Community Services.

What are the aims of the Partners In Recovery program?

PIR aims to better support you if you have a severe and persistent mental illness with complex needs, as well as carers and families. We do this by engaging with services and supports from multiple sectors to work in a more collaborative, coordinated and integrated way.

PIR also aims to build better links between various clinical and community support organisations and improve referral pathways between services.

We promote a community-based recovery model for people experiencing severe and persistent mental illness with complex needs.


What are the program objectives of the Partners In Recovery program?

The objectives of the PIR program are to:

  1. Facilitate better coordination of clinical and other supports and services to deliver ‘wrap around’ care individually tailored to the person’s needs.
  2. Strengthen partnerships and build better links between various clinical and support services responsible for delivering services to the PIR target group.
  3. Improve referral pathways that facilitate access to the range of services and supports needed by the PIR target group.
  4. Promote a community based recovery model to underpin all clinical and support services delivered to people experiencing severe mental illness with complex needs.
